Initial Path to Wellness Consultation
During your visit, we review all relevant aspects of your health & dietary history including your use of herbs, supplements, and medication along with past lab work.
Initial Consultations can be done in-person or remotely through telehealth
Your wellness consultation consists of two sessions:
1st Visit: 1 1/2 hours
The objective of our initial session is to gain insights into your unique health challenges. This includes reviewing a comprehensive intake form and questionnaires prepared before our session and an extensive interview. After our session, time will be spent to craft an wellness plan that is unique for you to be shared at the follow-up session.
Your wellness consultation may include any of the following depending on your own personal needs:
Dietary and Lifestyle Support – To achieve ideal weight and optimal wellness
Menu Planning – Creating a menu that meets your personal tastes
Budgeting and Shopping – Making the best choices within your budget
Suggested Supplemental, Botanical Extracts, Herbs – Their safe and effective applications
Suggested Labs – Nutritional evaluation of conventional and functional medical labs
Integrative Medicine – Collaborating with the appropriate doctors, therapists, trainers, and mind-body practitioners when appropriate
2nd Visit: 60 minutes
The goal of the second session is to share and adjust the designed program to suit your unique needs. Time will be spent discussing how to bring this plan into fruition.
Cost: $250.00 for clients who intend on paying out of pocket. Most Commercial Insurance Plans will cover your initial consultations. Please refer to the following link (Coming Soon - Website Under Revision) for more details on insurance reimbursements for your visits.

Insurance
Jay is currently “in-network” with Connecticut carriers including Aetna, Anthem BC/BS, Cigna, Connecticare, Harvard Pilgrim, and Medicare. He can provide a “superbill” for all “out-of-network” plans for possible reimbursement.
Most Insurance Plans are currently reimbursing Telehealth Visits at the same rate of In-Person Sessions. Some clients prefer to meet in person for the first visit before switching to telehealth due to its convenience. Generally speaking, most insurance plans will fully cover anywhere from three sessions to unlimited depending upon medical necessity. However sometimes a co-pay is required or deductible must be met. Please reach out to the office for further additional questions in regards to working with your plan.
